Job description

We are seeking a skilled Registered Nurse (RN) Case Manager to coordinate care for patients across the continuum of treatment. The RN Case Manager will assess patient needs, develop care plans, collaborate with providers, and help ensure safe, effective, and timely discharge or ongoing care.

With possible weekends. Travel experience preferred. No locals – must live 50+ miles away.
