A Field Nurse Case Manager is a registered nurse who coordinates and manages patient care outside of traditional hospital settings, often visiting patients in their homes, workplaces, or other facilities. They assess patient needs, develop care plans, monitor progress, and work with healthcare providers, insurers, and families to ensure effective treatment and recovery. Their goal is to optimize patient outcomes while efficiently utilizing healthcare resources, often focusing on workers' compensation, disability, or chronic illness cases.

How does a field nurse case manager typically coordinate care among multiple healthcare providers and patients?

Field Nurse Case Managers play a pivotal role in bridging communication between patients, healthcare providers, and insurance companies.
