Geriatric Home Health LVN
Amcare Pro Home Health, Palliative Care & Hospice is a mid-size home health care company with offices in East Texas and Dallas. We are currently interviewing for a PRN (transition to Full-Time upon eligibility & availability) LVN Field Nurse to cover the Kaufman, Scurry, Crandall, Terrell and surrounding areas.
We are looking for a Home Health LVN to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse. This role supports the delivery of safe, high-quality patient care through routine nursing visits, treatment administration, patient education, and timely documentation. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.
Responsibilities:
Provide skilled nursing care to home health patients under the supervision of an RN
Perform routine nursing visits including vital signs, medication administration, injections, and treatments
Assist with wound care and dressing changes per physician orders
Monitor patient condition and promptly report changes to the supervising RN
Educate patients and caregivers on medications, treatments, and safety
Accurately document all visits and care provided in the agency EMR
Ensure compliance with Medicare, Texas regulations, and agency policies
Coordinate visits and communicate effectively with the clinical team
Qualifications
1. Must currently be licensed in the state of Texas
2. Should have at least two years clinical experience
3. Prior Geriatric home health care experience is preferred but not required
4. Should have Reliable Transportation & Car insurance
5. Must be able to pass background check & have valid CPR & TB test/Chest x-ray
6. Bilingual is a PLUS
8. Authorized to legally work in the United States
7. Excellent compensation and mileage
