Job description

Hands-on Technical SME with deep implementation experience in FHIR-based healthcare interoperability. This role goes beyond consulting - you will architect, build, and validate FHIR integrations using SDK-level code across one or more languages (Python, .NET, Java, etc.), directly driving the evolution of our client's EHR connectivity solutions.
The ideal candidate is an engineer-first practitioner who understands clinical workflows, has written production-grade FHIR code, and can bridge the gap between standards specifications and working software.
Responsibilities -
Design, develop, and maintain FHIR-based integrations using SDKs such as HAPI FHIR (Java), firely-net-sdk (.NET), or fhir-py / Google FHIR (Python).
Write and review production-quality code for FHIR resource creation, parsing, validation, and transformation (STU3, R4, R4B).
Implement SMART on FHIR authorization flows, CDS Hooks, and Bulk Data Access in real-world integration environments.
Build and maintain FHIR capability statements, profiles (StructureDefinitions), valuesets, and implementation guides using tools like Forge, SUSHI/FSH, or IG Publisher.
Lead technical discovery sessions with EHR vendors and IT stakeholders to define FHIR endpoint capabilities, data mapping schemas, and API versioning constraints.
Communicate complex technical FHIR constraints to non-technical clinical stakeholders.
Architect and implement abstraction layers or 'FHIR Facades' to ensure integration logic remains EHR-agnostic and scalable across diverse clinical environments.
Implement and debug EHR-to-payer and EHR-to-HIN integrations across major platforms (Epic, Cerner/Oracle Health, Athenahealth, Veradigm), leveraging platform-specific FHIR APIs and sandbox environments.
Engineer touchless prior authorization (ePA) workflows via Da Vinci implementation guides (PAS, DTR, CRD), translating HL7 specs into deployable integration code.
Develop integration test suites, FHIR validators, and conformance testing pipelines to ensure standards compliance across environments.
Partner with engineering teams to translate clinical and business requirements into FHIR-based technical specifications, API contracts, and data flow diagrams.
Oversee technical connectivity strategy with HIEs and national networks (Carequality, CommonWell, Surescripts), contributing to both design and implementation.
Collaborate with Product teams to assess feasibility of interoperability features, providing SDK-level effort estimates and identifying standards gaps.
Mentor junior engineers on FHIR concepts, SDK usage patterns, and healthcare data modeling best practices.
