Social Worker
St. Paul, MN | Full-time

A skilled nursing and long-term care facility in the St. Paul area is seeking a Social Worker to support the psychosocial needs of residents and their families across admission, care, and discharge. Prior long-term care experience is what matters most to us for this role.

What you'll do:

  • Conduct psychosocial assessments on admission and at required intervals

  • Develop and update care plans addressing residents' social and emotional needs

  • Coordinate discharge planning and connect residents and families with community resources

  • Advocate for resident rights and help resolve concerns and grievances

  • Support residents and families through adjustment, coping, and end-of-life care

  • Collaborate with the interdisciplinary team and contribute to MDS assessments

  • Maintain documentation and ensure compliance with state and federal social services regulations

What we're looking for:

  • Prior social services experience in a skilled nursing or long-term care setting. This is our top priority and the main factor in the offered rate.

  • Bachelor's or Master's degree in Social Work

  • Current Minnesota social work licensure (LSW, LGSW, LISW, or LICSW)

  • Working knowledge of long-term care regulations, MDS, and discharge planning

  • Strong interpersonal, communication, and organizational skills

Compensation and benefits:
