We’re Looking For

A compassionate, dedicated RN Case Manager who makes patients feel at home—whether they’re in their own home, an assisted living facility, or a skilled nursing home. If you’re a hospice or home health Jedi with a valid GA RN license, driver’s license, and at least one year of experience, we want to meet you.

Your Role, Should You Choose to Accept It

  • Be a steady source of comfort and support to patients and families

  • Help families understand disease progression, self-care, and nutrition

  • Collaborate with the interdisciplinary hospice team to create and follow patient care plans

  • Manage symptoms like a boss and advocate for patient comfort
