A Family Model Provider is an Independent Contractor. Independent Contractors are not employees and are not eligible for company benefits.
Do you have room in your home and heart to support an adult or child with Intellectual and/or Developmental Disabilities? Are you able to support an individual to be successful at home, work and in the community? If so, you may find fulfillment in the role of a Family Model Provider.
We are looking for caring individuals who share our dedication to providing a supportive environment for persons with intellectual and /or Developmental Disabilities (I/DD) in their home. We ask that you bring a desire to serve others and share your home, and RHA will provide the training and support to ensure your success.
RHA Health Services is a provider of comprehensive, person-centered services for individuals living with I/DD. RHA is pleased to offer high-quality Family Model services throughout Tennessee. Family Model offers adults and children with I/DD the option to live in a natural home setting with a professional caregiver to support them in reaching personal goals and living the life they choose. RHA professional caregivers report that their lives and the lives of their families are enriched by this opportunity. RHA is an industry leading provider with decades of experience serving individuals with I/DD and our Family Model services are supported by a strong team of professional and administrative staff. We are typically able to make Family Model placements quickly and support individuals with a wide variety of needs.
As a Family Model caregiver you will support individuals with disabilities in a home setting. Level of supports are based on the needs of the individual in your care. Responsibilities include, but are not limited to:
  • Seeking to understand the individual in the context of their personal history, their social and family networks, and their hopes and dreams for the future.
  • Supporting the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self-care activities, leisure/recreation, education and working. Supports are provided according to the person's abilities and interests while ensuring privacy and dignity.
  • Preparing, teaching and assisting the person in food preparation, and cleaning of the home.
  • Honoring the choices, preferences and culture of the person you support.
  • Assisting in providing opportunities for social, emotional and spiritual needs to be met.
  • Assisting with transportation and scheduling of all medical, therapy, vocational and recreational activities.
  • Ensuring that the home environment is safe and in good condition.
  • Implementing services and supports to address goals identified through the Person Centered Plan. This includes medical, psychiatric, recreational, spiritual, and emotional needs.
  • Assisting the person supported by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
  • Participating in supervision as agreed upon in the RHA supervision contract.

RHA will assist you in the following capacities:
  • Provide support through a multidisciplinary team for crisis assistance and training as needed as you begin your journey as a Family Model caregiver. Training includes RHA's proprietary ProAct training, which focuses on supporting resiliency, emotional stability and problem solving.
  • Assist in coordinating respite care to support your needs as a caregiver.
  • Supervision will be provided to assist you in adjusting to your new role. The frequency and amount of supervision will be determined in a collaborative relationship with your RHA supervisor.
  • RHA's compensation package shall be determined based on the needs of the individual served and you will be paid as a contract provider.

Position Requirements:
• HS Diploma/GED
• 18 years old or older
• Documentation of home ownership: current mortgage statement or renter's lease
• Proof of homeowner's or renter's insurance or personal property insurance
• Valid Driver's License; Reliable transportation with valid vehicle insurance
• Ability to pass criminal background and Registry checks as well as a Drug Screen
• TB Screenings for all members of household
• Work as an Independent Contractor
Pre-employment screening:
  • Complete criminal background
  • Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
