• Collaborate closely with medical teams to ensure a holistic approach to patient care, offering insights into the psychosocial aspects of treatment.
• Facilitate connections with community organizations to arrange post-discharge services, guaranteeing a seamless transition for patients.
• Actively participate in hospital committees and provide social work expertise that contributes to policy development and care processes.
• Deliver crisis intervention and emotional support to patients facing challenging circumstances.
• Develop, implement, and monitor tailored discharge plans, aligning resources with patient needs for ongoing support after transitioning out of care.
• Maintain accurate documentation in compliance with hospital policies and relevant legal requirements, ensuring clear communication among team members.

You will primarily work with patients and families dealing with end-of-life care challenges within a hospice center. Understanding the unique emotional landscape of this patient population is crucial, as each individual may have varying needs related to grief, coping, and transition. • Master's degree in Social Work from an accredited program.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
