Key Responsibilities

As a Licensed Clinical Social Worker at Calhoun Families First Center, your responsibilities will include:

• Conducting thorough psychosocial assessments to identify the emotional, social, and financial needs of patients.
• Collaborating with interdisciplinary teams to create comprehensive care plans that address patients' psychosocial needs.
• Providing individual and group counseling to support patients coping with adjustment, grief, and mental health challenges.
• Coordinating with community organizations to facilitate post-discharge services, ensuring long-term continuity of care.
• Offering emotional support and education to patients and families regarding treatment options, resources, and coping strategies.
• Participating in hospital committees to provide expertise on social work-related issues and advocating for patient needs.
• Delivering crisis intervention and emotional support to patients and their families during challenging times.
• Documenting patient interactions and care plans accurately and timely, adhering to hospital policies and legal standards.

Patient Population / Client Focus

Your work at Calhoun Families First Center will center on a diverse patient population, including children and adults facing various psychosocial challenges related to health conditions, hospitalization, and transitions into community settings. Qualifications

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of medical terminology and social work ethics.
• Knowledge of mental health disorders, crisis intervention techniques, and best practices in psychosocial care.
• Exceptional communication and interpersonal skills, with an emphasis on empathy and collaboration.

Preferred Experience

While not mandatory, candidates with two or more years of experience working in a healthcare environment will be given preference. Familiarity with electronic health records (EHR) systems and patient management software will be a strong asset for efficient documentation and patient management.

Compensation and Benefits
