Key Responsibilities

• Conduct thorough psychosocial assessments to identify each patient’s emotional, social, and financial needs.
• Offer individual and group counseling to help patients cope with adjustment disorders, grief, and other related issues.
• Work collaboratively with our medical teams, ensuring that psychosocial interventions are integrated into patient care plans.
• Coordinate with external community organizations to facilitate post-discharge services, ensuring continuity of care.
• Provide emotional support and educational resources to patients and their families regarding treatment options.
• Participate in departmental and hospital-wide committees, advising on social work practice and policy initiatives.
• Develop and implement comprehensive discharge plans, linking patients with appropriate community services.
• Maintain accurate and timely documentation in alignment with hospital policies and legal regulations.

Patient Population / Client Focus

Your primary focus will be individuals and families from diverse backgrounds coping with a range of health-related challenges. You will play a critical role in addressing their unique psychosocial needs, ranging from chronic illness management to navigating difficult treatment decisions.

Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• Current active state licensure as a Licensed Clinical Social Worker (LCSW) in Georgia.
• Strong understanding of clinical methodologies as applied in a healthcare setting.

Preferred Experience

• A minimum of two years of experience working in a healthcare or hospital environment is preferred, providing a robust understanding of medical terminology and patient care processes.
• Experience in crisis intervention and familiarity with mental health disorders is an asset.
• Proficient communication and interpersonal skills are essential for effective collaboration within interdisciplinary teams.

Compensation and Benefits
