The Monroe County Health Department is seeking a compassionate, organized, and community-focused nurse to conduct health education programs as directed and aligned with Indiana’s Health First Indiana core public health services, support clinical public health nursing services, and assist with surges in communicable and non-communicable disease case investigations.
This position will play a vital role in addressing chronic disease prevention by delivering evidence-based programs, education, clinical services, community outreach, and care coordination designed to improve community health outcomes and ensure access to core public health services.
Essential Duties and Responsibilities
Clinical Services
· Administer vaccines according to current ACIP recommendations, screening procedures for contraindications and precautions, immunization record review, standard precautions, Indiana immunization requirements, Vaccines for Children (VFC) program standards, Indiana Department of Health vaccine storage and handling protocols and Rights of Medication Administration
· Utilize Children and Hoosier Immunization Registry Program (CHIRP) to review vaccine histories, verify immunization status, enter administered vaccines, and generate immunization reports and school compliance records
· Conduct health screenings including blood pressure, A1C, blood glucose, and cholesterol
· Perform blood lead level testing and case management for children with elevated blood lead levels (BLL)
· Participate in vaccine inventory management, temperature monitoring, vaccine accountability, and reconciliation activities in compliance with VFC and 317 program requirements
· Serves as a Primary Vaccines for Children (VFC) or back-up Coordinator, as assigned
· Assist with preparation of clinical patient visits including patient intake, documentation, cleaning, and restocking supplies
· Provide education regarding chronic disease prevention, obesity prevention and management, healthy nutrition and physical activity, and vaccine-preventable diseases and immunization schedules
· Conduct 8-week family programming focused on physical activity and nutrition, bi-annually
Community Health Outreach
· Participate in community events, mobile health services, health fairs, and outreach programs as assigned and based on community needs
· Provide evidence-based programming and education on infection prevention, fall prevention, obesity prevention, nutrition and healthy eating, and vaccine-preventable diseases as assigned and based on department and community needs
Communicable Disease Prevention & Response
· Serve as back-up for communicable disease case investigations and contract tracing as assigned and based on department and community needs
· Assist with outbreak response and public health surveillance activities as assigned and based on department and community needs
· Educate individuals, schools, and community partners on disease prevention strategies
Clerical & Office Support
· Assist with tracking vaccine and medical supply inventory
· Pull and organize medical records and patient intake forms
· Answer phones and assist with scheduling patient appointments
· Run and review immunization records utilizing CHIRP
· Enter and maintain accurate patient and program records
· Maintain accurate patient health records and public health documentation
· Maintain compliance with state and federal reporting requirements
· Track, collect, and submit Health First Indiana Key Performance Outcome data and monthly department activity data
