Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our growing Home Health team in Los Angeles County. This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.

As a Home Health Physical Therapist, you will work with senior care populations, including geriatric patients recovering from illness, injury, or surgery. You'll deliver skilled therapy services in home-based, assisted living, and community care settings, helping patients regain independence and improve their quality of life.

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient's needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.

Key Responsibilities:
  • Conduct in-home physical therapy evaluations and reassessments
  • Develop and implement personalized rehabilitation care plans
  • Improve strength, balance, mobility, and functional outcomes
  • Provide therapy in home health, assisted living, and community care settings
  • Educate patients and families on fall prevention and safe mobility techniques
  • Document visits using WellSky (Kinnser) EMR
  • Collaborate with interdisciplinary home health teams

Qualifications
  • Licensed Physical Therapist (PT) – California (Required)
  • CPR Certification (AHA approved)
  • Minimum 1 year of clinical experience (Home Health preferred)
  • Experience in senior care, geriatrics, rehab, or community-based care preferred
  • Strong clinical, communication, and patient education skills
  • Valid driver's license, reliable transportation, and auto insurance
