Samaritan Counseling, LLC is seeking a dedicated and experienced Senior Licensed Clinical Social Worker to join our team in a remote capacity. In this full-time role, you will utilize your expertise to address the psychosocial needs of seniors and their families within the Colorado Springs area. • Conduct in-depth assessments of seniors’ psychosocial needs, factoring in emotional, mental, and social influences on their overall well-being.
• Collaborate with clients and their families to create personalized care plans that reflect individual goals and preferences.
• Provide counseling and emotional support, assisting seniors and families as they navigate transitions such as relocation, bereavement, or health changes.
• Advocate for clients' rights and accessibility to community resources, ensuring they receive comprehensive support and guidance.
• Engage with interdisciplinary teams—comprising nurses, caregivers, and other healthcare professionals—to coordinate care effectively.
• Conduct regular virtual check-ins and home assessments to monitor clients’ well-being, reassess their living conditions, and address any safety concerns.
• Facilitate communication among family members involved in senior care, helping navigate conflicts and promote a collaborative approach to care.
• Educate clients and their families on available community resources, support groups, and programs to enhance social engagement.
• Maintain accurate and timely documentation of client interactions, assessments, care plans, and progress notes in our electronic health record (EHR) system.
• Stay abreast of evolving social work practices, policies, and regulations to ensure adherence to ethical standards.

Your primary focus will be on seniors and their families, helping them cope with age-related challenges, health issues, and life transitions. Your work directly impacts their emotional stability and quality of life, providing them with the tools necessary for emotional wellness. By connecting them to community resources and support systems, you will enhance their overall health outcomes.

• Master's degree in Social Work from an accredited institution.
• A current and valid Clinical Social Worker licensure (LCSW) in the state of Colorado.
• At least two years of relevant experience in a social work role, ideally in community health or a geriatric-focused setting.
• In-depth knowledge of social work principles, aging-related issues, and the community resources available to seniors.
• Exceptional assessment, counseling, and problem-solving skills.
• A strong ability to empathize, nurture, and build rapport with clients and their families.
• Excellent communication and interpersonal skills for effective collaboration with multidisciplinary teams.
• Proficiency in using electronic health record systems and various computer applications for documentation and reporting tasks.

• Experience working in a community health clinic or similar environment addressing the needs of senior populations.
• Demonstrated capability in crisis intervention and conflict resolution amongst family dynamics.
• Familiarity with telehealth platforms and remote service delivery methods.

The salary for this position ranges from $70,000 to $100,000 annually, commensurate with experience and qualifications. In addition to competitive pay, we offer:
