Shift: 5×8 Evenings (2:00 PM – 10:00 PM)

Job Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to join a 42-bed Long-Term Care (LTC) facility in Newark, NY. This opportunity is ideal for both experienced travelers and first-time travel nurses. The unit provides care for adult and geriatric patients with conditions including Dementia, Arthritis, Congestive Heart Failure (CHF), and Cerebrovascular Accident (CVA/Stroke).
 

Requirements
  • Active LPN/LVN License
  • BLS Certification (Required)
  • Minimum 1 year of LTC, Skilled Nursing, or Rehab experience
  • Epic EMR experience required
  • First-time travelers are welcome
Responsibilities
  • Provide direct nursing care to adult and geriatric patients in a Long-Term Care setting.
  • Administer medications, treatments, and monitor patient conditions.
  • Perform patient admissions, assessments, vital signs, intake/output monitoring, and clinical documentation.
  • Provide wound care, pressure ulcer prevention, dressing changes, and Wound VAC management.
  • Assist with Foley catheter care, blood glucose monitoring, feeding tube care, oxygen therapy, and specimen collection.
  • Maintain infection control practices and isolation precautions.
  • Collaborate with physicians and interdisciplinary teams including Physical Therapy, Pharmacy, Radiology, Social Services, and Unit Secretaries.
  • Educate patients and families while ensuring compassionate, high-quality care.
