ABOUT THIS POSITION

The Memory Care Coordinator is responsible for oversight of the memory care neighborhood including monitoring, training, and coordinating the programming and care services for the memory care neighborhood.ย ย 


Essential Responsibilities:

  • Protects resident protected ย ย ย ย ย health information (PHI) and adheres to Community confidentiality ย ย ย ย ย agreement.
  • Holds ย ย ย ย ย team members accountable to exemplifying core values and appropriately ย ย ย ย ย addresses behavioral issues as needed.ย 
  • Knowledge ย ย ย ย ย and understanding of state regulations and Company policies and ย ย ย ย ย procedures, ensuring compliance with both, as it relates to ย ย ย ย ย responsibilities.ย 
  • Actively ย ย ย ย ย and appropriately participates in state surveys and if necessary, assists ย ย ย ย ย with the development and implementation of the Plan of Correction.
  • Purchases ย ย ย ย ย needed supplies within the Health Services Budget and documents on Budget ย ย ย ย ย Spend-Down Sheet.
  • Purchasing ย ย ย ย ย within budget utilizing the expense management tools (budget spend-down ย ย ย ย ย sheet)
  • Communicates ย ย ย ย ย budget status weekly at designated stand-up meetings to include expenses, ย ย ย ย ย staffing hours, overtime, and agency projections/use.ย 
  • Maintains ย ย ย ย ย strict confidentiality of all known sensitive information to include ย ย ย ย ย residents, staff, and community operations.ย 
  • Attends ย ย ย ย ย and participates in the Daily Stand-up Meeting

Available by phone 24/7 and makes appropriate arrangements, when not availableย 

  • Leads ย ย ย ย ย and/or participates in investigations and documentation as needed.ย 
  • Participates ย ย ย ย ย on the marketing team and helps promote the Community through positive ย ย ย ย ย relationships with outside agencies and local organizations.ย 
  • Responds ย ย ย ย ย to, leads when necessary, and carries out responsibilities during ย ย ย ย ย emergencies/disasters per the Community's Emergency/Disaster Plan.

Participate in monthly all staff meetings and conducts in-services under direction of HSD.

Adheres to and conveys the community's philosophy of maintaining elements of resident directed care, individualized service and engagement plans and encouraging independence and engagement.

Is an active member of the leadership team and works collaboratively with the team including the Intergenerational Montessori Director (IMD).ย 

  • Recruits, ย ย ย ย ย hires, orients, trains, supervises, schedules and manages all MC medication ย ย ย ย ย assistants and care partners.ย 
  • Acts as a supervisor to the memory care health services team ย ย ย ย ย and ensures policies are procedures are followed.
  • Ensure ย ย ย ย ย completion of orientation and skills checklist.ย 
  • Responsible ย ย ย ย ย for the performance of Health Services Staff in MC providing ongoing ย ย ย ย ย feedback and coaching/counseling when appropriate to include leading or participating ย ย ย ย ย in employee terminations as necessary.ย 
  • Schedules ย ย ย ย ย & ensure sufficient staffing of care partners & medication ย ย ย ย ย assistant according to/in compliance with the staffing budget and the ย ย ย ย ย needs of the residents
  • Covers ย ย ย ย ย call-ins and tracks attendance of health services staff. ย 
  • Notify ย ย ย ย ย the Health Services Director and Executive Director when staffing budget ย ย ย ย ย is not sufficient to meet residents' needs to evaluate need for ย ย ย ย ย increase.ย 
  • Fulfils ย ย ย ย ย role of medication assistant, care partner or help with resident care as ย ย ย ย ย necessary.ย 
  • Regularly ย ย ย ย ย visits the Community during all shifts to build relationships and ย ย ย ย ย encourage employee engagement, supervision, and address performance ย ย ย ย ย concerns.ย 
  • Use ย ย ย ย ย staffing agencies only with approval of ED & Ownership Group, on a ย ย ย ย ย limited time basis. Receives pre-approval from the Executive Director and ย ย ย ย ย reports on Agency use weekly to include use and projections. Reviews and ย ย ย ย ย confirms invoices.ย 
  • Collaborating ย ย ย ย ย with residents and those involved with their care to develop ย ย ย ย ย Individualized Care Plans under the direction of HSD.
  • Takes ownership of the memory care ย ย ย ย ย neighborhood helping to ensure the neighborhood runs smoothly, with a ย ย ย ย ย positive work environment with high satisfaction and low employee ย ย ย ย ย turnover.
  • Helps ensure regulatory compliance including survey readiness ย ย ย ย ย and appropriate responses to state inspections.ย 
  • Works in collaboration with the community Director of Health ย ย ย ย ย Services and Executive Director to plan and assign work duties and assignments ย ย ย ย ย for the health services staff.
  • Monitors the day-to-day work provided by the health services ย ย ย ย ย staff; supervising, critiquing, suggesting, and educating the staff on ย ย ย ย ย proper work standards.
  • Routinely audits resident charts to ensure accurate service ย ย ย ย ย plans and postings are current and paperwork is complete and filed ย ย ย ย ย correctly.
  • Coordinates the resident's move-in (including paperwork with ย ย ย ย ย the Director of Health Services) and orients new residents and families to ย ย ย ย ย the memory care neighborhood.
  • Facilitates and coordinates the move-out process for all ย ย ย ย ย residents who are moving out from the memory care neighborhood. This ย ย ย ย ย includes assisting the resident, family, and other healthcare providers to ย ย ย ย ย make this transition as easy as possible.
  • Monitors and implements the service plan process, including ย ย ย ย ย the alert charting. This includes ย ย ย ย ย updating service plans on a routine and timely basis, scheduling and ย ย ย ย ย facilitating service plan meetings 6 months after move in or when needed ย ย ย ย ย with residents, employees, and families.
  • Routinely monitors the process to develop new and better ย ย ย ย ย ideas for the delivery of services and satisfaction of the residents. Listens to ย ย ย ย ย residents, family members, the care team and others for continued ย ย ย ย ย improvement and innovation. Works towards resident and legally ย ย ย ย ย authorized representatives high satisfaction.ย 
  • Monitors the delivery of services during all shifts by ย ย ย ย ย scheduling routine monthly visits to the late evening and night shift ย ย ย ย ย health services team.

Work directly with contracted pharmacy to ensure contracted services are promptly and appropriately provided. Report contract discrepancies to Health Services Director & Executive Director as appropriate.ย 

Coordinate physician orders and assist with pharmacy orders, when necessary.

  • Assists and helps facilitate the monthly memory care ย ย ย ย ย neighborhood staff meetings.
  • Organizes and assists with developing meetings with the memory ย ย ย ย ย care resident/family council.
  • Organizes and assists with facilitating the monthly ย ย ย ย ย Alzheimer's Support groups.
  • Monitors and coordinates the housekeeping duties with the ย ย ย ย ย housekeeping department as needed.ย 
  • Properly utilizes the software ย ย ย ย ย systems and devices used by the Community.

Follow infection control procedures, as established by the community. Wash hands after caring for a resident.

Report all incidents/accidents, regardless of how minor, to the Health Services Director as soon as possible. Follow mandatory abuse reporting and assist the Director of Health Services in investigations. Immediately report emergencies or potential liabilities to the Director of Health Services.ย 

Report all hazardous conditions and equipment to the Health Services Director or Executive Director.

Participate in all shift huddle meetings immediately after incidents or when requested.

Communicate complaints, suggestions and concerns to the shift supervisors, Director of Health Services, Executive Director or in suggestion box.ย 

Possess the ability to deal tactfully with employees, residents, family members, visitors, and the general public. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning.

Supervises, motivates, and evaluates the care staff to ensure that the highest quality of service is provided for residents.

To perform other duties that may be set forth by the Owners or specified by the Executive Director or, completing all assigned duties which may change from time to time according to resident's needs, staffing levels and working circumstances.

Coordinates with AD & IMD to plan, organize and implement resident activities that reflect the interests of the residents in cooperation with appropriate resident support groups and community departments in conjunction with the Activities Director and IMD.

Consults and works with other departments in developing appropriate activities as they relate to the physical, emotional, spiritual, intellectual, and social needs of the individual residents in conjunction with the Activities Director and the IMD.

Coordinates resident activities and programs in conjunction with the Activities Director, and the Intergenerational Montessori Director, if applicable which are sponsored by community groups. Develops plans and coordinates community volunteer program in conjunction with the Activities Director and the IMD.

Works closely with other departments to maximize level of independence and engagement for community residents.

Performs any other job duties as assigned by Director of Health Services or Executive Director.

Qualifications

  • Must be 18 years of age
  • LPN/LVNย 
  • Meet the Memory Care requirements for criminal history screening
  • Pass Abuse Registry Check (where applicable)
  • Pass required drug/alcohol screenings
  • First Aid and CPR Certification (where applicable)
  • Have adequate education, relevant training, or experience to provide for the needs of the residents


Physical Requirements

  • This is a physically active position. Ability to walk throughout community continually throughout the day
  • Ability to spend long periods of time on feet and to ambulate quickly
  • Ability to push and pull wheelchairs and other equipment up to 300 pounds
  • Ability to physically assist residents weighing 70 - 290 pounds
  • Visual and auditory acuity within normal limits
  • Bending, kneeling and occasionally reach above the shoulder level and carry up to 30 pounds
  • Ability to use office equipment
