Job description

Hospice Licensed Professional Music Therapist

Salary: $31.25-33.65/hr BOE

Schedule: Part-time| 3 days per week

Location: Toledo, Ohio and surrounding area

Requirements of the Music Therapist role:

  • Licensed Music Therapist (LPMT) valid in the state of Ohio.

  • Bachelor’s or Master’s degree in Music Therapy from an accredited institution with active state licensure or certification as required by local regulations.

  • Excellent interpersonal and communication skills, with the ability to build rapport with patients and families.

  • Comfortable with travel to patient homes, nursing facilities, or hospice centers as required by the role.

  • Strong understanding of the psychological, emotional, and physical aspects of end-of-life care.

  • Functional ability to incorporate various musical instruments, musical and therapeutic modalities.

About the Music Therapist role:

  • The Music Therapist will use music-based interventions to address the emotional, psychological, physical, and spiritual needs of patients on hospice services.

  • Provide music therapy sessions to hospice patients in individual or group settings, based on the patient's specific needs and goals.

  • Provides music therapy 

  • Use music as a therapeutic tool to help manage symptoms such as pain, anxiety, depression, and agitation, enhancing the quality of life for patients.

  • Offer emotional support to families and caregivers through music, facilitating meaningful connections and moments of peace during difficult times.

  • Conduct assessments of patients' physical, emotional, cognitive, and social needs to develop individualized care plans.

  • Collaborate with hospice nurses, social workers, chaplains, and other team members to ensure a holistic approach to patient care.

  • Offer grief support through music therapy for family members and caregivers before, during, and after the patient's death.

  • Maintain detailed records (using EMR system) of music therapy interventions, progress, and patient responses in accordance with hospice care documentation standards.

  • Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as needed.

  • All other duties as assigned
