Preceptor Home Health & Hospice is seeking an experienced Director of Clinical Services, Home Health to provide strategic and operational leadership for our clinical team. In this key leadership role, you will oversee all home health clinical services, ensure regulatory compliance, support staff development, and help drive exceptional outcomes for patients and their families.
What You'll Do
As the Director of Clinical Services, Home Health, you will lead the delivery of clinical services while ensuring compliance, quality, and operational success. Responsibilities include:
  • Providing overall direction and oversight of home health clinical services
  • Ensuring compliance with all applicable state and federal home health regulations
  • Coordinating and overseeing all direct and indirect patient care services
  • Managing the development, implementation, and review of individualized patient care plans
  • Collaborating with physicians and interdisciplinary team members to coordinate patient care
  • Supporting and mentoring clinical managers and staff to promote professional growth and performance improvement
  • Assisting with budget preparation, financial planning, and resource management
  • Monitoring operational indicators, census trends, staffing needs, and productivity metrics
  • Overseeing recruitment, onboarding, performance evaluations, and employee development
  • Ensuring accurate maintenance of clinical records, reports, and documentation
  • Managing inventory, supplies, and equipment required for patient care services
  • Leading orientation, training, in-service programs, and continuing education initiatives
  • Driving quality assurance and performance improvement activities across the organization
  • Assisting in the development and implementation of organizational goals, policies, and procedures

  • Serving as acting Home Health Executive Director in the Executive Director's absence

What We're Looking For
  • Current licensure in the State of Wisconsin as a Registered Nurse (RN), Physician, Physical Therapist, Occupational Therapist, Speech-Language Pathologist, or Social Worker
  • Three years of management experience in home health, hospice, or a related healthcare setting preferred
  • Demonstrated leadership experience supervising clinical and administrative personnel
  • Strong knowledge of home health regulations, compliance requirements, and healthcare operations
  • Excellent interpersonal, leadership, coaching, and team-building skills
  • Ability to analyze operational and financial data to support informed decision-making
  • Strong verbal, written, and organizational communication skills
