Position Overview

Community Health Network is seeking a compassionate and dedicated Clinical Social Worker (LCSW) to join our team in providing essential support and services to individuals and families within our hospice center. This full-time remote position offers the opportunity to make a meaningful impact in the lives of patients facing end-of-life challenges, ensuring they receive the emotional and psychological care they deserve.

Work Environment

This position is fully remote, allowing for flexibility while adhering to the core functions required within a hospice setting. As a Clinical Social Worker, you will connect with patients and their families through virtual platforms, ensuring continuity of care while fostering a supportive environment. Regular working hours will be complemented by occasional evening and weekend commitments for specific events and programs.

Key Responsibilities

  • Provide individualized and group counseling sessions, employing evidence-based therapeutic approaches tailored to the unique challenges faced by patients nearing the end of life.
  • Collaborate with a multidisciplinary team to develop and implement personalized service plans, ensuring alignment with each patient's goals and preferences.
  • Advocate relentlessly for patient rights, access to necessary resources, and important social justice issues that affect vulnerable populations dealing with terminal illnesses.
  • Engage actively in program development and evaluations, contributing insights for grant reporting and enhancing community resources.
  • Facilitate communication and coordinate efforts with community organizations, government agencies, and volunteers to optimize service delivery.
  • Participate in ongoing training, team meetings, and community outreach initiatives to maintain a high standard of care and support.
Patient Population / Client Focus

The focus of this role is primarily on individuals and their families navigating the complexities of hospice care. This includes providing emotional support during critical transitions, assisting in coping strategies related to grief, and ensuring that patients' psychosocial needs are met through compassionate counseling and advocacy.

Qualifications
  • Education: A Master’s degree in Social Work from an accredited institution is required.
  • Licensure: Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • Skills: Strong communication, empathy, and organizational skills are essential for effectively supporting patients and their families in a hospice setting.
Preferred Experience
  • Previous experience working in a nonprofit organization or similar healthcare environment is highly desirable, with a preference for experience specifically related to hospice and palliative care.
  • Knowledge of trauma-informed care principles, social advocacy, and community resources will be beneficial in fulfilling the responsibilities of this role.
Compensation and Benefits
