This position is primarily onsite within our home care setting, fostering a collaborative atmosphere that encourages teamwork and communication among professionals across various specialties. You will work with a diverse patient population, encountering a variety of orthopedic conditions, and employing best practices in physical therapy to promote recovery in a supportive environment.

• Conduct thorough physical therapy evaluations for patients, assessing both their physical abilities and functional limitations.
• Develop and implement individualized treatment plans based on clinical findings, utilizing evidence-based practices tailored to the unique needs of each patient.
• Regularly reassess and document patients' progress, adjusting treatment plans as necessary to ensure optimal recovery and goal attainment.
• Maintain accurate clinical documentation, including SOAP notes and progress summaries, utilizing our state-of-the-art electronic record system efficiently.
• Educate patients and their families about rehabilitation processes, setting realistic expectations and discussing potential challenges faced during recovery.
• Meet or exceed minimum utilization goals and contribute to other clinic objectives, ensuring a balance between patient care and clinic performance.
• Collaborate with colleagues across different therapy disciplines to coordinate comprehensive care for clients receiving multiple services.
• Promote and embody the core values of Evolution Therapy, serving as an ambassador for our compassionate and patient-centered culture.

Your work will primarily focus on individuals recovering from orthopedic injuries, surgeries, and chronic conditions affecting mobility. You will engage with a wide array of patients, inclusive of various ages, races, and ethnic backgrounds, diagnosed with conditions that require specialized care. The approach is not just about treating the physical aspects but also addressing the psychosocial factors influencing recovery.

• A Bachelor’s degree in Physical Therapy from an accredited program is required, with a Master's or Doctorate in Physical Therapy preferred.
• Current, unrestricted license to practice Physical Therapy in Florida, complying with state regulations.
• Demonstrated commitment to providing both in-person and telehealth evaluations and treatments, adapting as necessary for patient needs.
• A passion for working with a diverse patient population and a strong commitment to ethical and inclusive care practices.
• Adaptability to a dynamic work setting, embracing changes and challenges head-on.
• A collaborative mindset, supported by a focus on customer service and teamwork.

• Experience working within a multidisciplinary clinic or therapy setting is advantageous.
• Prior experience with telehealth protocols and remote patient engagement is a plus.
• Familiarity with the latest rehabilitation technologies and methodologies will enhance the treatment experience.

We offer a competitive salary range of USD 80,000 to 100,000 annually, commensurate with experience.
