Nurse Practitioner/Physician Assistant Primary Care
Job Summary
The Nurse Practitioner/Physician Assistant will provide primary care services to patients in an assisted living/skilled nursing facilities. The successful candidate will work collaboratively with the facility's medical director and other healthcare professionals to provide high-quality care to patients.
Responsibilities
  • Conduct initial and ongoing assessments of patients' health status
  • Develop and implement care plans based on patients' needs and goals
  • Prescribe medications and treatments as appropriate
  • Order and interpret diagnostic tests
  • Provide patient education and counseling on health promotion and disease prevention

Qualifications
