Nurse Supervisor (RN/LPN) - PM/NOC Shift

Location: Ahwatukee Post Acute - Phoenix, AZ
Position Type: Full-Time
Shift: PM or NOC Shift (Schedule TBD)

Position Summary

Ahwatukee Post Acute is seeking an experienced and compassionate Nurse Supervisor (RN or LPN) to provide clinical leadership during evening or overnight operations. The Nurse Supervisor is responsible for overseeing nursing staff, ensuring quality resident care, supporting admissions, conducting rounds, managing clinical concerns, and serving as the clinical leader for the facility during assigned shifts.

This position plays a critical role in maintaining regulatory compliance, supporting staff development, and ensuring residents receive high-quality, person-centered care in a skilled nursing environment.

Essential Duties and Responsibilities
  • Provide leadership and supervision to nursing staff during assigned shift.
  • Conduct routine rounds throughout the facility to monitor resident care, safety, and staff performance.
  • Coordinate and assist with admissions, readmissions, transfers, and discharges.
  • Respond to resident, family, physician, and staff concerns in a timely and professional manner.
  • Ensure physician orders are implemented accurately and timely.
  • Monitor documentation for accuracy, completeness, and regulatory compliance.
  • Assess residents with changes in condition and coordinate appropriate interventions.
  • Serve as the clinical resource and decision-maker for the facility during assigned hours.
  • Assist with staffing coordination and assignment adjustments based on resident needs and census.
  • Follow up on incidents, accidents, falls, and significant clinical events.
  • Ensure infection prevention and control practices are followed.
  • Collaborate with department managers to promote quality outcomes and resident satisfaction.
  • Participate in performance improvement initiatives and quality assurance activities.
  • Support and coach nursing staff through education, mentoring, and accountability.
  • Ensure compliance with federal, state, facility, and company policies and procedures.
  • Complete additional nursing management and supervisory duties as assigned.
Qualifications
  • Current Arizona RN or LPN license in good standing.
  • Minimum one (1) year of nursing experience required.
  • Skilled nursing facility or long-term care experience preferred.
  • Previous charge nurse, nurse supervisor, unit manager, or leadership experience preferred.
  • Current CPR/BLS certification.
  • Strong clinical assessment and critical thinking skills.
  • Ability to lead teams, manage priorities, and make sound decisions independently.
  • Excellent communication and customer service skills.
Physical Requirements
  • Ability to stand, walk, bend, lift, and assist residents as needed.
  • Ability to respond quickly to emergencies and resident care needs.
  • Ability to work in a fast-paced healthcare environment.
