Senior Care Solutions is seeking a Full-Time Medicare Field Registered Nurse (RN) to join our growing home health team and provide high-quality, patient-centered care throughout Broward County.

Location: Broward County, Florida
Job Type: Full-Time
Setting: Medicare Certified Home Health

Position Overview

The Medicare Field RN is responsible for providing skilled nursing services to Medicare patients in their homes. This position requires strong clinical judgment, excellent communication skills, and the ability to work independently while coordinating care with physicians, therapists, and other members of the healthcare team.

Responsibilities
  • Perform comprehensive nursing assessments and develop individualized plans of care.
  • Provide skilled nursing services in accordance with the physician’s plan of care.
  • Complete accurate and timely clinical documentation.
  • Monitor patient progress and communicate changes in condition to the appropriate members of the care team.
  • Educate patients and caregivers on medications, disease management, safety, and home care needs.
  • Coordinate care with physicians and other healthcare professionals.
  • Perform medication management and reconciliation as appropriate.
  • Recognize and report changes in patient condition.
  • Provide wound care and other skilled nursing services within the scope of practice.
  • Ensure compliance with Medicare, AHCA, and agency requirements.
  • Maintain accurate documentation in the agency’s electronic medical record system.
  • Provide compassionate, professional care while promoting patient independence and safety.
  • Current and unrestricted Florida RN license.
  • Previous Medicare-certified home health experience.
  • Strong knowledge of Medicare home health requirements and documentation.
  • Ability to complete OASIS assessments.
  • Excellent clinical, organizational, and communication skills.
  • Ability to work independently and manage a field-based caseload.
  • Reliable transportation and a valid driver’s license.
  • Ability to travel throughout Broward County and surrounding areas.
