We are committed to helping patients recover from disabling medical events - including strokes, brain or spinal cord injuries, major orthopedic injuries, and medically complex illnesses - through personalized and intensive therapy programs. Our rehabilitation team combines expertise and compassion, providing tailored physical therapy, occupational therapy, and speech therapy to address each patient's specific challenges and goals. We are seeking a Pharmacist to join our team of passionate patient caregivers!

Qualifications
Required Skills:
  • Doctor of Pharmacy from an ACPE accredited college of pharmacy
  • At least one year of hospital pharmacy experience
  • Supervisory experience preferred.

Additional Qualifications/Skills:
  • Strong analytical, problem-solving, written communication and interpersonal skills
  • Must be flexible with work schedule
  • Dependable attendance and job performance
  • Licensed or license-eligible in the State of Texas

Responsibilities
  • Ensures safe transitions of care by completing comprehensive medication reconciliation and final verification of admission and discharge orders for all patients.
  • Evaluate and verify WellSky physician's orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration and therapeutic redundancy
  • Clarifies physicians' orders, when necessary, through direct and timely contact with prescriber or designated practitioner
  • Demonstrates knowledge with antimicrobial stewardship , which includes therapy optimization, de-escalation, and selection of appropriate, evidence-based antimicrobial regimens.
  • Assists with monitoring and appropriate prescribing of anticoagulation therapy
  • Provides patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staff
  • Supervises pharmacy technicians in activities related to medication dispensing, ordering and inventory management.
  • Thorough documentation of all clinical interventions
  • Exemplifies the Ernest Health mission, vision and values and acts in accordance with Ernest Health policies and procedures
  • Uphold departmental, and institutional, policy as appropriate
