Job Description
Profiles Needed: 9/5 (up to 8 candidates)
Position: Family Nurse Practitioner (FNP) - Emergency Department & Ambulatory Care
Contract Type: Non-personal services
Period of Performance: 10/01/2025 - 09/30/2026, with option year through 09/30/2027
Location: Crownpoint Healthcare Facility, Crownpoint, NM
(Assignments may include Emergency Room, Adult Ambulatory Care, Thoreau Health Clinic, Pueblo Pintado Clinic)
Shift Details:
โ€ข ED shifts: 10-12 hours, typically starting 8:30 or 9:00 a.m. (daily, including weekends/holidays)
โ€ข Clinic hours: 7:30 a.m.-6:00 p.m. or 8:00 a.m.-4:30 p.m.
โ€ข No on-call required
โ€ข Must be flexible for additional hours if needed
Scope of Work:
โ€ข Provide urgent/acute care in ER Fast Track
โ€ข See adult outpatients in ambulatory clinic
โ€ข Same-day clinic coverage at Thoreau or Pueblo Pintado (all ages)
โ€ข Manage diabetes, hypertension, chronic care needs, refills, referrals
โ€ข Perform minor procedures (laceration repair, splinting, I&D, toenail removal)
โ€ข Enter referrals and documentation in EHR
โ€ข Complete charts within 72 hours (pay may be held if records are incomplete)
โ€ข Attend daily 10 a.m. ER staff huddles
โ€ข May assist with triage, COVID-19 testing, and vaccinations
Qualifications:
โ€ข Graduate of accredited NP program
โ€ข Unrestricted U.S. state NP license and DEA
โ€ข Board certified (AANP or ANCC)
โ€ข Current BLS, ACLS, PALS (PALS strongly preferred)
โ€ข Experience across ED and outpatient care preferred
Support:
โ€ข Physicians always available in ED for consultation
โ€ข Support staff: RNs, MAs, NAs, MDs/DOs (staffing dependent)
โ€ข Specialty services at Crownpoint: optometry, dental, speech, PT, inpatient unit, women's health, pediatrics
โ€ข No specialty services at Thoreau or Pueblo Pintado clinics
Housing/Travel:
โ€ข Government housing may be available, subject to vacancy
โ€ข Otherwise, provider responsible for housing, per diem, and travel
