The Role

Our client, a multidisciplinary outpatient rehabilitation team in Hesperia, California, is seeking a Physical Therapist to join their ambulatory care programme. Working under the supervision of the Director of Operations, you will serve as the clinical expert in movement disorders, delivering comprehensive physical therapy services to a diverse patient population spanning orthopaedic, neurological, and cardiac conditions, as well as pre- and post-surgical rehabilitation.

This is an excellent opportunity for a new graduate or early-career PT to build their skills within a collaborative, multilingual team that has been serving its community for over eight years. You will work closely with primary care providers and specialists to ensure seamless, patient-centred care transitions.

Key Responsibilities

  • Perform patient examination, evaluation, physical therapy diagnosis, prognosis, and outcome assessment across a broad ambulatory caseload
  • Develop and implement individualised treatment plans for patients with orthopaedic, neurological, cardiac, and pre/post-surgical conditions
  • Deliver direct PT interventions including therapeutic exercise, manual therapy, neuromuscular re-education, and functional mobility training
  • Provide consultation, education, and clinical guidance to patients, families, and the broader care team
  • Lead and contribute to programme planning, implementation, evaluation, and staff education training activities
  • Maintain accurate and timely clinical documentation in the EMR in accordance with facility and payer requirements
  • Direct and supervise PTAs, rehab aides, and support personnel as assigned
  • Collaborate with primary care providers and specialists to support smooth care transitions and optimise patient outcomes
  • Contribute to quality improvement and maintain compliance with applicable regulatory and accreditation standards

Requirements

Must-Have ★

  • ★ Graduate of an accredited Physical Therapy professional programme (Bachelor's degree minimum)
  • ★ California Physical Therapist licence — required upon hire
  • ★ BLS certification (National Provider — American Heart Association) — required upon hire
  • ★ New graduate or entry-level experience in an ambulatory care facility or medical group practice setting
  • ★ Experience with programme planning, implementation, evaluation, and education training

Preferred / Nice-to-Have

  • Master's degree or PhD in Physical Therapy — advantageous
  • Minimum 1 year of outpatient physical therapy clinical experience
  • Familiarity with CARF and Title XXII standards
  • EMR documentation experience

Compensation

  • Base salary: $107,660 – $167,107 per annum — placement within range based on experience
