Job Type: Full-time | Variable 12-hour shifts and rotating Weekends and holidays per month

Up to $20,000 Sign-on Bonus

How you'll contribute

A Registered Nurse (RN) who excels in this role:

  • Coordinates the Patient Care Team's delivery of personalized, efficient care with respect for patient's spiritual and cultural differences in accordance with the PA State Board of Nursing Licensure requirement and ANA Scope and Standards of Practice.
  • Provides patient assessment throughout patient stay. Participates in discharge planning while maintaining a therapeutic partnership with patient, family and caregivers.
  • Implements the interventions identified in the established plan of care and collaborate with members of the Patient Care Team.
  • Collaborates with physician as part of Interdisciplinary Team.
  • Is knowledgeable regarding patient condition, completes patient assessment and reassessment, reports changes to the Physician.
  • Demonstrates annual RN competencies and specialized competencies as identified.
  • Completes annual required education and CPR.
  • Obtains and maintains ACLS
  • Completes yearly medication and dysrhythmia examination with a passing score greater than 86% Maintains accountability and productivity for appropriate staffing following the flexible-staffing plan.
  • Attends and participates as a minimum requirement 6 staff meetings annually, as defined by Manager. Reads all staff meeting minutes.
  • Maintains proficiency in all aspects of Conemaugh Connect Bar Code medication administration, and in all aspects of Conemaugh Connect powered by Epic.
  • Participates in Process Improvement utilizing Lean Six Sigma methodology to improve patient outcomes. Active participant in a team pertaining to the current nursing strategic plan.
  • Commits to Common Ground Code of Conduct.
  • Actively participates in Patient Satisfaction initiatives.
  • Attends educational sessions as directed by the nurse manager to gain knowledge about new treatment options, procedures, or equipment.Timely renewal of PA State RN licensure.
  • Participates in own professional development maintaining required competencies unique to Nursing float team such as Death Pronouncement, HIV source counselling, hourly face to face assessment of patients demonstrating need for behavioral restraint.
  • Staffs the overflow unit in accordance with Capacity Management policy and procedure.
  • Initiates the electronic CORE Measure Assessment Tool and the appropriate daily CORE Measure Assessment documentation (Heart Failure, AMI, Pneumonia, Stroke, Surgical Care Improvement, VTE, and Immunization), assess patient daily.
  • Completes the Transition of Care; documentation to include the Physician Orders Sheet (Transition of Care for Home or PFC or Transition of Care for Skilled, Intermediate, LTAC OR Acute Care Rehab), discharge instructions and medication reconciliation. Completes the AVS.
  • Completes Transfer Consent to Acute Care Hospital, if the patient is being transferred acute.
  • Maintains yearly competency in Handle with Care.
  • Maintains competency in care of the peritoneal dialysis patient.
  • Works towards competency in NIH stroke assessment
  • Collaborates with nursing unit to maintain competencies specific to individual unit.
  • Maintains competency in IV therapy and blood administration. Proficiency in administering high risk medications.

What we're looking for
Applicants should have a current state RN license and possess an associates degree from an accredited nursing school. Additional requirements include:

  • BSN Preferred

  • 1-2 years minimum bedside RN experience required

  • 2 years acute medical-surgical care

  • BCLS

  • ACLS and PALS preferred

  • 2- 4 years critical care experience preferred

  • Basic Dysrhythmia preferred

  • Critical Care Course preferred

  • Trauma Course preferred

  • Stroke Course preferred

  • Experience in cardiac monitoring, neuro, critical care and IV Therapy preferred
