Position Summary
The Hospice RN Clinical Coordinator is responsible for carrying out administrative and management functions and oversight in the absence of the Director of Clinical Services. May carry caseload due to ebb and flow of patient census. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.
Position Specific Responsibilities
  • Promotes a positive working relationship between agency and corporate personnel.
    • Promotes positive working relationship with family, physician and other facilities.
    • Attends and participates in staff meetings, educational programs and community events, as requested.
    • Promotes Hospice philosophy.
    • Communicates identified needs and potential solutions to Hospice Director of Clinical Services.
    • Assists with onboarding, provide mentorship to new staff.
    • Assists with DME and medical supply orders to ensure quality and advise Director of Clinical Services of any trends.
    • Monitors supply closet for expiration dates.
    • Manages medication orders to ensure needed meds are available for any admissions. Fax to providers as needed.
    • Submits requests for medical records and hospital inpatient charts, and processes provider notifications when relevant.
    • Oversees scheduling of inpatient unit and ensures that schedules are submitted to staff 6 weeks prior per union contract. Ensure that staff are scheduled per matrix for census and acuity.
    • Answers incoming telephone calls / clinical calls, takes accurate messages, and transfers calls appropriately.
    • Assists DCS with Interdisciplinary Team meetings, including agendas, update coordination notes as needed.
    • Provides coverage for DCS as needed.
    • Actively participates in Quality Assessment and Performance Improvement (QAPI) activities.
    • Makes visits to the hospital as needed for additional educational support as well as assist the Clinical Liaison in admission transfers to IPU.
    • Performs RN Case Manager responsibilities as needed and may carry a caseload due to patient census ebb and flow.
    • Participates in on call duty as needed or as back up to on call nurse, performs patient admissions and visits as needed.
    • Performs other duties as assigned
Education and/or Experience
  • Active and unencumbered Registered Nurse License required.
  • One (1) year of Hospice experience required.
Skills
  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
Certifications, Licenses, and Registrations
  • Valid driver's license and auto liability insurance.
  • Current Registered Nurse State License as required.
  • Current CPR certification.
