Key Responsibilities
  • Collaborate with primary care providers to recognize, treat, and manage mental health and psychosocial issues, contributing to an integrated care model.
  • Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations, and make well-informed recommendations for treatment goals alongside the primary care team.
  • Provide brief yet impactful interventions for patients requiring immediate mental health services, tailoring strategies to individual needs.
  • Advise the primary care providers on which patients would benefit from additional specialty mental health resources.
  • Implement follow-up procedures to monitor patient progress, ensuring changes to treatment plans are made when necessary.
  • Develop relapse prevention plans in collaboration with patients, supporting them in maintaining stable functioning within their daily lives.
  • Identify at-risk patients and design strategic plans to prevent deterioration of their mental health status.
  • Coordinate health service delivery relating to behavioral health, creating connections with external treatment providers as needed, with patient consent.
  • Maintain accurate documentation of patient progress and diagnostic information in electronic health records.
  • Regularly communicate and collaborate with primary care providers to ensure that treatment plans meet the evolving needs of patients.
  • Facilitate behavioral change in patients who may also be facing physical health disorders by promoting healthier lifestyle choices.
  • Empower patients with self-management skills and provide educational resources that enable active participation in their treatment and recovery journey.
  • Consult with clinic management and other team members regarding behavioral services and contribute to program evaluation.
  • Support patients in adhering to medical treatment plans initiated by primary care providers, assisting with strategies to handle any medication-related side effects.
  • Uphold the Code of Ethical Conduct for Clinical Professionals and complete other duties as assigned.

Compensation and Benefits
