Key Responsibilities

  • Deliver concise and targeted therapeutic interventions, tailored to meet the mental health needs of patients.
  • Guide primary care providers in determining which patients would benefit from primary care treatment versus those requiring specialized mental health services.
  • Initiate follow-up measures to monitor patient progress and modify treatment approaches as needed.
  • Create relapse prevention strategies to ensure patients maintain stable functioning and effective coping skills.
  • Identify at-risk patients and work collaboratively on prevention plans to address worsening conditions.
  • Coordinate healthcare services within the behavioral health domain, establishing connections with community resources when necessary.
  • Maintain accurate records of patient progress and diagnostic information in treatment charts, ensuring compliance with agency regulations.
  • Communicate regularly with primary care providers about patient needs and treatment progress, facilitating collaborative care.
  • Support behavioral changes in patients with physical disorders, promoting healthier lifestyle choices through guidance and education.
  • Empower patients with self-management skills and provide them with educational materials to support their recovery journey.
  • Consult with clinic management and other team members on behavioral services and contribute to program evaluation efforts.
  • Assist patients in adhering to medical treatments prescribed by the primary care provider and develop coping strategies for any side effects experienced.
  • Uphold the Code of Ethical Conduct for Clinical Professionals and complete additional tasks as assigned.
Patient Population / Client Focus

You will work with a diverse array of patients, including veterans and their families, addressing mental health complexities that may arise from trauma, transition, and life stressors. Qualifications
  • A Master’s degree in Clinical Counseling from an accredited institution is required.
  • Possession of a current Licensed Professional Counselor (LPC) license is mandatory.
  • Proficiency in mental health assessment, treatment planning, DSM-IV diagnoses, and various therapeutic modalities including family therapy and group therapy.
  • Ability to communicate bi-lingually in English and Spanish is preferred, but not required.

Compensation and Benefits

The salary for this position ranges from USD 87,000 to USD 105,000 annually, commensurate with experience and qualifications.
