Job Description Summary
Vera Whole Health is seeking a proactive and compassionate National Board-Certified Health and Wellness Coach (NBC-HWC) to deliver personalized, evidence-based virtual coaching to help members achieve lasting behavior change, navigate healthcare resources, and meet their personal wellness goals. As a key member of our integrated care team, you'll collaborate with primary care providers, registered dietitians, behavioral health clinicians, and care managers to help patients achieve sustainable behavior change and improved health outcomes. This position will primarily support our multi-disciplinary Weight Management program.
How will you make an impact & Requirements
This is a remote position that can be based anywhere within the United States. Must be able to work M-F, 8 am-5pm PST.
Position Qualifications/Essential Functions:
Health Coaching Responsibilities
  • Conduct 30-minute, personalized 1-on-1 coaching sessions using motivational interviewing and evidence-based behavior change strategies to surface intrinsic motivation and co-create actionable SMART goals.
  • Facilitate engaging group coaching sessions and workshops to expand support networks and increase member participation.
  • Provide ongoing accountability, evaluate progress, and adapt strategies over time to drive long-term member success.
  • Administer appropriate screening tools and health measurements within scope of practice.
  • Maintain thorough and timely clinical documentation for each visit, providing members with customized care plans.

Care Team Collaboration & Member Advocacy
  • Partner with care managers, registered dietitians, and providers within structured weight management programs.
  • Serve as the subject matter expert in behavior change, providing insights and support to the broader multidisciplinary care team.
  • Understand individual health conditions and trend data to guide members to the appropriate level of care and help eliminate barriers to health.
  • Connect members with digital health tools, health plan benefits, employer wellness initiatives, and community resources.
  • Resolve member concerns with empathy, ownership, and professionalism.
  • Conduct proactive outreach calls to re-engage members and track ongoing participation..

Operations, Administration & Compliance
  • Manage scheduling, appointment coordination, and follow-ups to maintain an active member panel.
  • Participate in targeted wellness campaigns, incentive programs, and client events.
  • Strictly adhere to the NBHWC Code of Ethics, HIPAA, PHI/PII protection protocols, and IT security standards.
  • Maintain a dedicated, private, and professional workspace for virtual visits.

  • Actively engage in quality assurance reviews, call audits, and routine feedback to continuously elevate service standards.

Qualifications & Requirements Required:
  • National Board-Certified Health and Wellness Coach (NBC-HWC) credential.
  • Bachelor's degree in Health Science, Kinesiology, Nutrition, Psychology, or a related field.
  • 2+ years of health coaching experience, preferably within a clinical, healthcare, or digital health environment.
  • Bilingual English/Spanish or multilingual fluency
  • Experience with weight management

Experience & Skills
  • Certified Diabetes Prevention Program (DPP) Lifestyle Coach, preferred
  • Experience working with patients on GLP-1s, preferred
  • Experience delivering care via telehealth or working within a remote team structure preferred.
  • Strong baseline knowledge distinguishing the clinical boundaries between health coaching, health education, and behavioral health/therapy.
  • Proven customer service excellence with a track record of driving patient engagement and retention.
  • Technical proficiency with Electronic Medical Record (EMR) systems, Salesforce, and Google Workspace preferred.
