The position is fully remote, enabling you to work effectively from your chosen environment while connecting with clients within our hospice center. • Provide clinical services to clients facing symptoms of severe emotional disorders and major mental illness, both in individual and group settings.
• Develop integrated care plans that ensure access to necessary resources, while facilitating communication among clients, family members, and a multidisciplinary care team.
• Conduct evidence-based screenings and assessments, following best practices to identify clients' unique needs and tailor treatments accordingly.
• Engage in crisis intervention strategies while offering support to clients in moments of acute distress.
• Carry an active caseload, ensuring timely and effective treatment while monitoring client progress and adjusting care plans as required.
• Collaborate with colleagues to achieve common goals, leveraging diverse perspectives to enhance service delivery.
• Maintain ethical standards and uphold the organization’s code of conduct in all client interactions and administrative duties.
• Participate in on-call coverage to address urgent client needs during and after program hours.
• Contribute to agency committees aimed at improving the quality of services provided and advancing operational goals.

You will be primarily focused on supporting clients in hospice care who are grappling with complex emotional and psychological challenges. This population often requires specialized approaches that incorporate understanding grief, anxiety, and depression. Your work will play a crucial role in helping these individuals find peace and comfort during a critical time in their lives.

• Master's Degree in Social Work from an accredited institution.
• Valid state licensure as a Clinical Social Worker (LCSW) in California.
• A minimum of two years of direct clinical experience in a mental health setting.
• In-depth knowledge of mental health diagnoses, interventions, and treatment modalities suitable for clients facing severe emotional difficulties.
• Proficient in documentation and case management in compliance with established standards.

• Experience in hospice or palliative care environments.
• Familiarity with trauma-informed care practices and evidence-based therapeutic approaches.
• Previous involvement in community outreach initiatives aimed at underserved populations.
• Ability to work independently while remaining engaged with a remote team.

The annual salary for this position ranges from USD 70,000 to 85,000, commensurate with experience and qualifications.
