This role is primarily remote, allowing you to connect with patients from the comfort of your home while promoting work-life balance. • Conduct thorough bio-psychosocial evaluations to identify psychosocial challenges and develop effective treatment plans.
• Utilize evidence-based treatment modalities in individual and group therapy settings to promote effective patient outcomes.
• Consult with medical and support staff to enhance comprehensive care programs, fostering better treatment adherence and coordination.
• Regularly assess and document patient progress and treatment effectiveness in accordance with departmental standards.
• Educate patients and their families on medical and social work recommendations aimed at restoring their mental health and functioning.
• Coordinate advocacy efforts within the community to support patients’ self-determination and access to resources.
• Stay current on community resources and clinical best practices to continuously improve patient care.
• Attend relevant trainings and professional development activities to enhance clinical expertise.
• Uphold the highest standards of social work practice according to the National Association of Social Workers (NASW) Code of Ethics.

You will primarily work with individuals requiring support for stress, anxiety, depression, trauma, and other psychosocial issues.
