• Conduct thorough biopsychosocial evaluations to identify patients’ needs and barriers to effective treatment.
• Develop individualized treatment plans, utilizing evidence-based therapeutic modalities and both individual and group therapy techniques.
• Collaborate with a multidisciplinary team to ensure comprehensive care tailored to the unique circumstances of each patient.
• Document all social work activities in patients' medical records in accordance with departmental standards, ensuring clarity on the patient's history and progression.
• Educate patients and their families about mental health conditions and treatment options to facilitate compliance and enhance understanding.
• Act as an advocate for patients’ and families’ needs, connecting them with community resources to strengthen their support systems.
• Maintain current knowledge of community resources to assist patients effectively and promote self-determination in client systems.
• Fulfill departmental reporting requirements and contribute to case-related statistics to help optimize operational outcomes.
• Engage in ongoing professional development and training relevant to clinical responsibilities to enhance therapeutic skills and efficacy.
• Adhere strictly to ethical standards in social work practice as outlined by the NASW Code of Ethics.

As a Licensed Clinical Social Worker at Clear Sky, you will serve a diverse patient population, ranging from adolescents struggling with mental health issues to adults facing family dynamics and chronic illnesses. Your work will significantly contribute to the recovery and well-being of individuals and families who seek help in navigating their emotional and mental health challenges.

• A Master’s Degree in Social Work from an accredited institution and a current license issued by the State Board of Social Work Licensure.
• Must possess a Licensed Clinical Social Worker (LCSW) credential.
• Demonstrate in-depth knowledge and skills necessary to address the needs of patients across various stages of life and cultural backgrounds.

• Clinical experience in a health or mental health setting, with a focus on the populations served in Rosedale and surrounding areas.
• At least six months to one year of clinical experience, showcasing an ability to adapt therapeutic interventions to meet specific patient needs.
• Familiarity with community resources and existing support networks beneficial to enhancing patient outcomes.
