This position is set in a remote work environment, allowing for greater flexibility while engaging with a diverse patient population through our Community Health Clinic. Although remote, the LCSW will collaborate closely with healthcare professionals, patients, and families to provide comprehensive psychosocial support. Understanding the unique challenges and barriers faced by our patients, particularly those from underserved communities, is central to delivering effective care.

• Independently manage a caseload of patients, with a focus on identifying and addressing psychosocial barriers that affect their treatment and recovery.
• Conduct thorough evaluations of patients’ social, emotional, cultural, and financial circumstances to inform their treatment plans, ensuring that care is personalized and comprehensive.
• Provide leadership in care coordination discussions, addressing complex issues such as advance directives and treatment decisions, especially for high-risk patients.
• Implement crisis intervention strategies when necessary, offering immediate support to patients and their families during times of distress.
• Work alongside case managers and the healthcare team to educate and connect patients and families with necessary resources and referrals, ensuring their psychosocial needs are met.
• Supervise and mentor staff including Social Workers, MSWs, interns, and volunteers, fostering a collaborative learning environment.
• Respond promptly to psychosocial service requests, ensuring efficient resolution and adherence to treatment plans, thus minimizing delays in discharge and follow-up care.
• Participate in specialty team rounds and patient care conferences, contributing valuable insights regarding social work interventions and recommendations based on your specialized knowledge.
• Plan care coordination efforts, particularly for pediatric patients with complex discharge needs, ensuring comprehensive follow-up with community resources.

The ideal candidate will work with a diverse range of patients within various demographic and socioeconomic backgrounds, particularly those facing significant psychosocial challenges. By addressing these multifaceted issues, the LCSW will empower patients to take charge of their own health decisions, thereby enhancing overall patient satisfaction and outcomes.

Applicants must possess a current and valid LCSW license in California. A Bachelor’s degree in Social Work or a related field is required, along with a minimum of 2 years of relevant clinical experience. The candidate should demonstrate a comprehensive understanding of therapeutic practices and psychosocial support methodologies.

While 2 years of clinical experience is mandated, candidates with experience in a hospital-based or community health setting, particularly working with underserved populations, will be given preference. Knowledge of healthcare systems, advanced social work practices, and supervisory experience will significantly enhance your application.

This position offers a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience.
