A-Line Staffing is now hiring a Pharmacist (RPH) – Urgent Need. This will be full time 30 Hours/Week. Schedule: Friday, Sat, Sun, Monday | 3:00 PM – 1:00 AM Pay Range - $63.00-65.00 an hour Benefits – active after 90 days of employment Key Responsibilities Review patient profiles for drug interactions, sensitivities, and medication appropriateness Verify and dispense prescription medications accurately Perform final dispensing quality checks Resolve prescription issues and physician clarifications Handle External Adjudication (EA) queue resolutions Assist with therapeutic interventions and physician outreach Communicate with patients regarding medication questions and concerns Maintain compliance with quality assurance and controlled substance procedures Support hazardous waste drug disposal procedures Collaborate with pharmacy staff to ensure operational accuracy and efficiency Required Qualifications Active Pharmacist license in good standing Bachelor’s Degree in Pharmacy or PharmD required At least 1 year of pharmacy experience in retail or long-term care setting INDKS Delivering outreach and care management services to chronic care patients Partnering care providers to coordinate care and further develop partnerships Improve the care management program Ensure coordinated patient centered care Provide care coordination for members Develop care plans, collaborate with physicians and other health care professionals in implementing and maintaining care plan Manage the course of care, care plans and all communications in the EMR Travel through the continuum of care Addressing patient care and community needs Track care coordinator visits to members Assist with linkage to care coordination Make recommendations for alternative medical care for approval by the primary care physician Carry out the plan of care Demonstrate health care informatics competency in order to manage patient care, outcome data and current and emerging health care technology Optimizing communication among care providers, promoting seamless transitions between care settings and facilitating the appropriate use of health care resources Providing care coordination to patients during transition from acute care to the next level of care in the continuum Work in collaboration with the primary care physician, staff and the patient to ensure care coordination occur across the continuum of care Attend inpatient family meetings and care conferences when necessary to integrate inpatient care with patients' longitudinal, intra-disciplinary care plans Promote communication and collaboration to keep all care team care members informed of patients'care needs and to ensure that patients access and benefit maximally from available health care resources Trigger a review of the care plan
