Job description

Looking for a nursing role where you can build real connections with patients and be part of their day-to-day progress?

Chestnut Health Systems is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for our Residential Services team in Bloomington, IL. This role offers the opportunity to work more closely with patients in a structured, supportive environment while collaborating with a full interdisciplinary team.

This is a role where you are not rushing from patient to patient. You are part of the process.

Responsibilities
  • Complete admission assessments and assist in establishing initial care plans
  • Support discharge planning to ensure continuity of care
  • Administer medications and monitor for effectiveness and side effects
  • Coordinate treatment with an interdisciplinary team including medical, behavioral health, and support staff
  • Provide integrated service coordination to support whole-person care
  • Carry out health care plans designed by Practitioners
  • Provide health education to patients and their families
  • Assess patients in-person for acute and ongoing health needs
  • Serve as a liaison between prescribers, pharmacies, and other healthcare providers
  • Promote a safe, therapeutic environment for all patients
  • Uphold Chestnut’s standards for customer service and patient care

Qualifications
  • High school diploma or equivalent required
  • Active Illinois RN or LPN license required
  • Knowledge of pharmacology and medication side effects required
  • CPR certification required (or ability to obtain within 90 days of hire)
  • Basic computer skills required, including EMR systems
  • Must successfully complete Chestnut’s background check process
Compensation

The anticipated starting pay for this position ranges from $19.85 to $40.84 per hour, depending on licensure (LPN or RN), experience, and qualifications.

Chestnut considers multiple factors when determining starting pay, including skills, education, experience, local market conditions, and internal equity.

Opportunities for additional earnings may include shift differentials and bonuses, where applicable.
