Based in Albuquerque, we are passionate about expanding our services to rural areas of New Mexico to ensure access to comprehensive and compassionate end-of-life care. Guided by the belief that every individual—regardless of financial circumstances or geographical location—is entitled to dignified, holistic healthcare, we are committed to supporting patients and families during their most challenging moments.

Position Overview: We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated team. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. Responsibilities:

Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
