Job description

Responsibilities

The Scheduling Coordinator assists in participant care responsibilities by performing receptionist duties, escorting participants to exam rooms and scheduling and coordinating various appointments between the participant and providers. The Scheduling Coordinator also performs a variety of clerical duties and provides administrative support to all clinic functions.
Participant Scheduling and Support – 75%

  • Schedules appointments with internal providers and with external network specialists’ offices for consultations and procedures as ordered by PACE primary care physicians.
  • Assists in prioritizing the scheduling of participants by consulting with medical staff, on a daily basis, to manage the participant flow.
  • Works as a liaison to these specialists’ offices for coordinating care and relaying information to PACE operations leadership team as appropriate.
  • Coordinates with the transportation department to schedule outside and special in-house appointments for participants.
  • Ensures that appointment information is relayed to the appropriate departments and caregivers as necessary in addition to notifying participants and their families of appointments and transportation arrangements.
  • Coordinates escorts with the ADHC staff.
  • Takes information from participants to start the triage process for those seeking care without a scheduled appointment


Clinic and Support – 25%

  • Greets and checks in participants for clinic appointments, performs any necessary documentation in medical records, and performs computer entries to update participant information.
  • Answers clinic telephone calls and directs calls and messages to appropriate parties or voice mail as requested.
  • Performs data entry of participant information, diagnostic orders, test results and data PACE information per established procedures.
  • Maintains inventory of clinic supplies and puts needed supplies on re-order list. Ensures that necessary supplies are always available.
  • Performs any necessary filing or clerical duties necessary to complete charting requirements.
  • Completes weekly audits as assigned.
  • Confirms that lab results are received and contact the lab company as needed for additional information.
  • Requests consultation notes and diagnostic tests by fax as needed.

REQUIRED

  • 1+ yr administrative or clerical experience that includes filing, record keeping, data entry and providing customer service
  • 1+ yr scheduling experience
  • Medical Terminology
  • EMR

PREFERRED

  • Using Epic EMR
  • Current state issued Certified Nursing Assistant (CNA) license and/or Medical Assistant certificate
  • Clinic health care experience in a geriatrics setting
  • Experience as a Nursing Assistant in home care, long-term care, daycare or group home
