What You'll Do:

  • Manages health conditions and illnesses by developing an individualized plan of care.
  • Complete the palliative face to face visit requirements prior to the third and subsequent palliative benefit periods to assess hospice eligibility based on the palliative conditions of participation and the local coverage determinations.
  • Provide respectful, compassionate care in patients' homes.
  • Work in collaboration with the interdisciplinary team.
