Early Intervention Coordinator / Case Management Specialist

White Glove Community Care is a growing healthcare organization providing home care and Early Intervention services. We are committed to delivering high-quality services to the children and families we serve while building strong teams and providing opportunities for professional growth.

We are currently seeking an experienced Early Intervention Coordinator / Case Management Specialist to join our team.

Position Details

Schedule:
Monday–Thursday: 9:00 a.m. – 5:00 p.m.
Friday: 9:00 a.m. – 12:30 p.m.

Reports To: Early Intervention (EI) Manager

About the Role

The Early Intervention Coordinator / Case Management Specialist will work closely with the EI Manager to support the day-to-day operations of the Early Intervention department.

We are specifically looking for a candidate with previous Early Intervention experience who understands the workflow, documentation, compliance requirements, and time-sensitive nature of EI services. The ideal candidate is highly organized, detail-oriented, dependable, and able to independently manage multiple priorities in a fast-paced environment.

Key Responsibilities

  • Create and submit referrals through EI Hub.

  • Open, manage, and maintain cases in ProviderSoft.

  • Download, upload, and maintain required case documentation.

  • Monitor and update authorizations, plans, and case statuses.

  • Maintain transition logs and follow up with Service Coordinators regarding outstanding items.

  • Manage case queues, assignments, and authorization tracking.

  • Schedule IFSP meetings with Brooklyn Regional.

  • Generate and distribute certificates to coordinators and providers.

  • Prepare and send certified correspondence to families when required.

  • Review session notes for accuracy, completeness, and compliance.

  • Run and maintain weekly and monthly EI reports, including case status, authorization, progress, and outstanding-item reports.

  • Assist with audits and ensure documentation meets applicable regulatory and agency requirements.

  • Identify outstanding or time-sensitive cases and escalate concerns to the EI Manager.

  • Maintain consistent communication and follow-up with Service Coordinators, providers, families, and internal departments.

  • Perform additional EI-related administrative and case-management responsibilities as assigned by the EI Manager.

Qualifications & Experience

  • Previous Early Intervention experience is strongly preferred.

  • Experience working with EI Hub, ProviderSoft, or similar Early Intervention systems is highly preferred.

  • High School Diploma required; Associate's or Bachelor's Degree preferred.

  • Knowledge of Early Intervention case workflows, authorizations, documentation, and compliance requirements.

  • Strong computer and Microsoft Office skills.

  • Excellent organizational and time-management abilities.

  • Strong written and verbal communication skills.

  • Ability to manage multiple cases, deadlines, and competing priorities.

  • Strong attention to detail and follow-through.

  • Ability to work independently while also collaborating effectively with the EI Manager and broader team.

  • Professional, dependable, and comfortable working in a fast-paced, deadline-driven environment.

We are looking for someone who can step into the role with an existing understanding of Early Intervention and become a dependable resource to the EI Manager and department.

The successful candidate will be proactive, accountable, organized, and committed to ensuring cases are followed through from beginning to end. This position requires someone who does not wait for issues to arise but actively monitors cases, follows up on outstanding items, and keeps the EI Manager informed of concerns.
