Nurse Practitioner – Hospital Medicine / Skilled Nursing Facilities Iberia Medical Center – New Iberia, Louisiana

Iberia Medical Center is seeking a dedicated and compassionate Nurse Practitioner to join our Hospital Medicine team in New Iberia, Louisiana.

This unique dual-role position provides patient care across both the acute hospital setting and affiliated skilled nursing/long-term care facilities. The ideal candidate will be adaptable, clinically strong, and committed to delivering high-quality, patient-centered care across multiple care environments.

Hospital Medicine Responsibilities
  • Conduct comprehensive evaluations and assessments for hospitalized patients.
  • Develop and implement individualized treatment plans.
  • Provide medication management and monitor patient responses.
  • Collaborate with physicians, nurses, and multidisciplinary teams to ensure coordinated patient care.
  • Maintain accurate and timely documentation within the electronic health record (EHR).
Nursing Home / Skilled Nursing Facility (SNF) Responsibilities
  • Provide routine, acute, and transitional care for patients in nursing home and skilled nursing settings.
  • Perform initial and follow-up visits, including post-hospital discharge evaluations.
  • Manage chronic conditions and acute changes in patient status.
  • Coordinate care plans with nursing staff, therapy teams, and facility administration.
  • Conduct medication reconciliation and adjust treatments as appropriate.
  • Participate in care plan meetings and communicate regularly with patients' families as needed.
  • Collaborate with hospital teams to support smooth transitions of care between the hospital and nursing facilities.
