The ECM Care Coordination Specialist supports Casa de la Familia's Enhanced Care Management (ECM) program through CalOptima. This position serves as a primary point of contact for members and assists with outreach, engagement, appointment coordination, resource linkage, and documentation activities. The ECM Care Coordination Specialist works closely with Care Managers, healthcare providers, and community organizations to help members access medical, behavioral health, and social support services.
