Position Summary

As a Dual-Diagnosis / Dual-Service Specialist, you’ll play a vital role in supporting individuals with co-occurring challenges, such as mental health and reentry needs. You’ll be part of a compassionate team dedicated to creating lasting change by connecting individuals to the resources and opportunities they need to achieve stability and independence.

Key Responsibilities

In this role, you will:
- Provide direct support to individuals navigating dual-diagnosis challenges, ensuring their unique needs are met.
- Collaborate with a multidisciplinary team to create tailored care plans that address mental health, reentry, and other critical areas.
- Act as a bridge to community resources, helping individuals access education, housing, employment, and life skills programs.
- Foster meaningful connections between individuals and the Golden Age Foundation ecosystem of care.
- Monitor progress and adapt support strategies to ensure long-term success.

Required Skills and Qualifications

We’re looking for someone who brings:
- A minimum of 1 year of experience working in a related field, such as mental health, social services, or community support.
- Strong interpersonal and communication skills to build trust and rapport with individuals from diverse backgrounds.
- A collaborative mindset and the ability to work effectively within a multidisciplinary team.
