Job description

Field Supervisor Responsibilities:
The Field Supervisor will be primarily responsible for ensuring that policies and procedures of the In-home meet the standards for proper care of clients, training of staff, and general program integrity.
• Schedules initial visits with clients to complete required admission and enrollment paperwork.
• Schedule an initial meet and greet with client and direct service worker, upon arrival there should be an introduction to the client of the DSW, in-home training with the DSW, tour around the house, and tutorial of how to read the clients binder. 
• Ensures that all documentation on visit reports is complete, accurate, and reflects the client’s plan of care.
• Selects and maintains qualified, organized, professional, and paraprofessional staff to provide care for the needs of the patient.
• Communicates with the RN of any staff that needs Medication Certification Training, or if the client has been admitted and discharged from the hospital or rehabilitation facility. 
• In the event of the absence of a DSW, the Field Supervisor ensures the responsibilities of the DSW are met and patient care is continued. Field Supervisor may also designate the schedule to fill-in in the field, as necessary.
• Evaluates the performance of DSW through yearly evaluations and onsite pop-ups check.
• Ensures that all required policies and procedures are implemented, enforced, and meet State guidelines.
• Is self-reliant and plans appropriately.
• Maintains high visibility and availability via telephone and/or while in the office, projecting a concerned, professional, attitude toward WCSS personnel.
• Performs all duties in compliance with State Guidelines.
• Attends WCSS in service and company both weekly and mandatory meetings as directed.
• Maintains and updates current patient binders by completing monthly reports.
• Maintains patient, caregiver, and WCSS confidentiality always.
• Many duties are performed in the office and out in the community. 
• Must have reliable transportation and be able to travel locally regularly.
• Rotate the on-call role as needed.
• Willing to become back-up staff in client’s home when there are call ins, or no show DSW’s.
• Maintain the clients schedule according to the CPOC and fill the clients home accordingly.
• Additional Responsibilities as assigned by CEO.
Special Requirements: Must have a vehicle with current insurance coverage and a valid state driver s license.
Minimum Position Requirements: (1) year experience providing training or direct services to persons with developmental disabilities, (1) year of supervising experience (1) year related experience. Along with the following abilities:
• Able to type 40+ wpm.
• Must be willing to learn new things.
• Be dependable and a fast learner.
• Able to communicate and act in a professional manner.
• Must be compassionate and caring.
• Obtain organizational skills.
• Must have some scheduling, recruiting, and marketing experience. 
• Must be able to travel to clients place of residence.
