Must have strong knowledge of CT Medicaid & Medicare, different kinds of Husky Insurances. Renewal process and state benefits.

Redeterminations, spend down, applications for title 19, reinstatement,

As a Social Worker, you'll contribute to our success in the following ways:

  • Ensures that all activities are performed aligned with the vision of Elara Caring's board of directors, executive team, and the leadership of the Home Health team.
  • Manage and navigate Medicaid and Medicare benefits and eligibility for clients.
  • Understand and work with different HUSKY plan options and requirements.
  • Collaborate with client case managers, conservators, and other care team members regarding benefits and coverage.
  • Partner with Department of Social Services (DSS) and navigate state systems to resolve client benefit issues.
  • Manage the Medicaid renewal process, including required documentation and follow-up.
  • Assist clients with accessing and maintaining other state benefits and resources.
  • Identify gaps or interruptions in coverage and work toward reinstatement of Medicaid/Medicare benefits.
  • Maintain accurate documentation and follow up with appropriate agencies and stakeholders to ensure benefits remain active.

What is Required?

  • Degree of Social Work from a school of Social Work accredited by the Council of Social Work Education
  • 1+ year of social work experience in a Healthcare Setting.
  • Current State License as a Social Worker (preferred)
  • Excellent verbal and written communication skills
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

You will report to the Clinical Team Manager.
