Job description

Hospice Clinical Liaison

The Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. The Hospice Clinical Liaison develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners. The ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process.

Qualifications & Skills
  • LVN, RN, or MSW required
  • 1โ€“2 years of healthcare marketing/sales experience preferred
  • Minimum of 2 years hospice experience highly preferred
  • Strong familiarity with local referral partners including:
    • Hospitals
    • Physicians
    • Board & Care (B&C)
    • Assisted Living Facilities (ALF)
    • Residential Care Facilities for the Elderly (RCFE)
    • Skilled Nursing Facilities (SNF)
  • Excellent written and verbal communication skills
  • Self-directed with the ability to work independently
  • Determined, collaborative, and strong work ethic
  • Consistent, persistent, patient, and compassionate interpersonal approach
  • Valid driverโ€™s license, clean driving record, and current auto insurance required
Key Responsibilities
  • Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners
  • Plan and coordinate marketing events, educational sessions, and community luncheons
  • Educate healthcare stakeholders on hospice and palliative care services
  • Support patients, families, and referral partners throughout the referral and admission process
  • Provide hospice informational / goals of care conversations with patients and families
  • Facilitate initial patient consent process
  • Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities
  • Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership
