Our ideal candidates have experience in providing quality medical services to their patients, assessing the state of the patient's health, performing routine procedures like providing medication and checking wounds, also write progress reports for doctors to review. Qualifications:

  • Graduate with a Bachelor's Degree in Nursing
  • Registered Nurse with an active license

Responsibilities:

  • Complete assessments of patient and family to determine home care needs.
  • Evaluates and performs initial and ongoing comprehensive assessments on patients referred for home health nursing services.
  • Establishes the plan of care in collaboration with the physician and the patient in accordance with physician orders and patient needs.
  • Coordinate and educate patients and families as needed regarding care.
  • Evaluate patient care and progress.
