About the Role:
ComForCare Home Health Care - Hilton Head is looking for a dedicated and compassionate Registered Nurse (RN) to deliver exceptional in-home care to patients in the Hilton Head, SC area. This is a rewarding opportunity to make a meaningful difference in the lives of patients and their families — right in the comfort of their own homes. Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized care plans for home health clients
  • Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting
  • Monitor and evaluate patient health status, documenting changes and communicating with physicians as needed
  • Educate patients and family members on disease management, medications, and home safety
  • Coordinate care with interdisciplinary team members including therapists, aides, and case managers
  • Ensure accurate and timely clinical documentation in compliance with home health regulations
  • Provide compassionate end-of-life and chronic disease management support as needed
Requirements:
  • Active Registered Nurse (RN) license in the state of South Carolina
  • Previous home health, hospice, or community nursing experience preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Reliable transportation and a valid driver's license for home visits in the Hilton Head area
