QUALIFICATIONS:

Registered Nurse with currentlicense in the state of Wisconsin/Illinois; One year of experienceas an RN in an acute care setting, preferably two years of experience. Home Careexperience is desired.

ESSENTIAL FUNCTIONS: (Other duties as assigned)
  •  Performs assessmentsand develops individualized patient care plans and managesmulti disciplinary care as applicable.
  •  Provides skilled nursing care in a patient's home as ordered by the attending physician in conjunction withthe plan of care.
  • Supervises LPN's and Home Health Aide's in a patient's home.
  • Complies with agency policies and procedures. Promotes and maintains an agency environment thatis in compliance with federal, state, and localregulations.
  •  Submits clear and appropriate documentation in a timely manner and documents patient education.
  •  Communicates effectively to physicians, supervisors, and coworkers in a timely manner.
  •  Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients.
    Participates in case conferences to promote coordination of care
  •  Promotes continuity of care with appropriate admissions, transfers and discharges.
  • Keeping abreast of developments in healthcare, attend in-service training in addition attend agency meetings as required.
  • Reports to appointments as scheduled.
  • Maintain a professional appearance
