Medical Doctor โ€“ Consultant, Care Coordination & Health Education

We are looking for a licensed Medical Doctor in California to join our team as a consultant, focusing on care coordination, approval of care plans, and direct client health education and support. The ideal candidate is culturally competent, client-centered, and passionate about empowering patients through clear communication, accessible health information, and collaborative care.


Responsibilities

Care Coordination & Plan Review

  • Provide consulting services to streamline patient care and ensure effective, culturally appropriate care plans.ย 
  • Review and approve care plans to ensure they meet medical standards and are tailored to each patient's individual needs, background, and circumstances.ย 
  • Collaborate with healthcare teams, case managers, and social service providers to coordinate care and improve patient outcomes.ย 
  • Conduct weekly consultations to provide clinical guidance and support to healthcare providers and care teams.ย 
  • Stay current on best practices in patient care, care coordination, and culturally responsive medicine.ย 

Client Engagement & Health Education

  • Engage directly with clients to explain their care plans, diagnoses, treatment options, and next steps in plain, accessible language โ€” free of medical jargon.ย 
  • Answer client questions with patience, clarity, and cultural sensitivity, ensuring individuals feel heard, respected, and informed about their own health and care.ย 
  • Provide health education support on topics relevant to each client's care, including chronic disease management, medication adherence, preventive care, mental health, and available community resources.ย 
  • Support clients in understanding how to navigate the healthcare system, including how to advocate for themselves and engage with their care team.ย 
  • Follow up with clients as needed to address ongoing questions, clarify medical information, or provide reassurance during complex care transitions.ย 


Requirements

  • Medical Doctor (MD or DO) licensed in the state of California.ย 
  • Completion of cultural competency training.ย 
  • Strong communication and interpersonal skills, with the ability to explain medical concepts clearly to clients of varying health literacy levels.ย 
  • Comfortable engaging directly with clients from diverse backgrounds, including those with limited English proficiency, lived experience with trauma, or distrust of healthcare systems.ย 
  • Passion for improving patient outcomes through collaborative, patient-centered, and culturally responsive care.ย 
  • Ability to work within a multidisciplinary team environment.ย 
  • Bilingual or multilingual candidates strongly encouraged to apply.ย
