Seeking Part-Time CNA Home Health Aide

Territory Includes: Baltimore City & County/Carroll/Howard

28 hours/week

8:00am - 4:30pm

Must have valid CNA, Driver's License, Own Reliable Transportation and Proof of Car insurance

Primary Duties:

  • Attends Agency regional office and team meetings asrequired. Attends in-service educational programs. Demonstrates flexibility in adjusting to changesin changes withprocedures and in daily schedule.Cleans direct patient carearea,as necessary.Identifiessafety hazards in the home and reports it to the team's Operations Director or primarycase-manager. Reinforces with patient/family ways to remove identified safety hazards in the home.

  • Documentservicesin accordance withAgency medical records and timekeeping policies, including the absence of patients from their homes. Documents patient progress reports clearly and concisely for multidisciplinary caseconference.Maintains effective working relationships with other departments andparticipatesin multidisciplinary quality and service improvement teams. Participates in meetings and on committees andrepresentsthe department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

  • Performs personal care activities (bathing, dressing, oralhygieneetc.). Performs other related activities (light meal preparation, personallaundryetc.).Assistswith transfers and range of motion exercise programs as instructed by professional staff. Performs procedures as delineated onHHA (Home Health Agency)Skills Competency Checklist as instructed by professional staff.

  • Recognizes signs of emotional distress (such as anger and depression). Encourages patient mobility and socialization during visits. Recognizes and responds to special needs of patients/families experiencing terminal illness.

  • Reportsidentifiedalterations in patient behavior or mental status to primary case manager or the team's Operations Director. Reportsidentifiedpatient care or family health or social problems to the team's Operations Director or primary case manager. Reports all problems in carrying out assigned activities to primary case manager or the team's Operations Director. Collaborates with case manager or professional staff in the revision and implementation of the Home Health Aide plan of care. Informs the team's Operations Director ofthe needto changethe patient'svisit schedule.

Qualifications:

  • Ability to read, write, and perform arithmetic calculations.

  • High School Diploma or GEDpreferred.

  • Home care experiencepreferred.

  • CNA (Certified Nursing Assistant) in Maryland.

  • CHHA (Certified Home Health Aide)in the District of Columbia, Maryland, Virginia, or any combination asrequiredbased on work location(s).

  • CPR (Cardiac Pulmonary Resuscitation).

  • Completion of MedStar Health's home health aidewounddiabetes and CHF competencies afterhire.

  • Valid State Driver's License in good standing with a dependable vehicle for transportation.
